FSR Tampa Area Sarcoidosis Awareness Walk
Kick off 2025 by supporting those living with sarcoidosis . This social meet-up and walk to raise funds for the Foundation for Sarcoidosis Research will start at 11 am and end at 2:30pm ET. *Photo Release*
All attendees and registrants to this live even hereby grant Foundation for Sarcoidosis Research (FSR) permission to use their likeness in a photograph in any and all of its publications, including website entries, without payment, or any other consideration. And hereby consent to and authorize the use and reproduction, in print or electronic format by FSR to use all and/or part of their story promotional and educational uses, without compensation. They agree that FSR may use said story about them with or without their name for any lawful purpose including, but not limited to publicity, illustration, advertising, and web content.
Furthermore, all attendees hereby irrevocably authorize FSR to edit, alter, copy, exhibit, publish, or distribute their story and photos for purposes of publicizing FSR’s programs or for any other lawful purpose. In addition, attendees waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, they waive any right to royalties or other compensation arising or related to the use of my story or photographs.
All attendees hereby hold harmless and release and forever discharge FSR from all claims, demands, and causes of action which the individual attendee, their heirs, representatives, executors, administrators, or any other people acting on my behalf, or on behalf of my estate, have or may have by reason of this authorization.

By registering for the Tampa Area Sarcoidosis Awareness Walk ("Event"), currently scheduled for March 8, 2025 11:00am-2:30pm ET at Anderson Park.
As a participant you agree to fully and completely waive and release the Foundation for Sarcoidosis Research ("FSR"), Tampa General Hospital, directors, officers, administrators, representatives and executors, past and present employees, volunteers, agents, supervisors, participants, all state and local governments, assigns, sponsors, their representatives and successors and other persons (collectively, the “Releasees”), from any and all claims, liabilities, damages, and/or causes of action arising out of an injury to you (or your dependent) and from any and all claims, liabilities, damages, and/or cause of actions arising from, or relating to, your (or your dependent’s) participation or attendance in the Event.
Inherent and Potential Risks
You understand that the Event may involve physical activity involving walking or playing games (like cornhole, volleyball, etc.). You understand that physical activity, by its very nature, carries with it certain inherent risks. You assume all risks associated with participating in the Event relating to the risk of strenuous physical activity associated with walking, collisions with other participants, vehicles, and pedestrians, injury from sports or falling. They acknowledge that they (or their dependent) may incur minor injuries, major injuries, and catastrophic injuries including paralysis and death. They assume all risks from contact with other participants and volunteers (including, but not limited to, contracting a disease), negligent or wanton acts of other participants and volunteers, any defects of conditions of the park (including uneven surfaces), failure of other participants, vehicles, and non-participants to observe traffic signals or laws, and the effects of weather including high heat,
thunderstorms, lightning, precipitation, cold temperatures, high winds, and/or humidity.
All Releasees agree that they consume any food at their own risk and r waive and release the Foundation for Sarcoidosis Research ("FSR"), Tampa General Hospital, directors, officers, administrators, representatives and executors, past and present employees, volunteers, agents, supervisors, participants, all state and local governments, assigns, sponsors, food donors, and their representatives and successors and other persons (collectively, the “Releasees”), from any and all claims, liabilities, damages, and/or causes of action arising out of an injury to you (or your dependent) and from any and all claims, liabilities, damages, and/or cause of actions arising from, or relating to, your (or your dependent’s) consuming any food or drink provided by the facility. They are solely responsible for any adverse health effects from food or beverage consumption, regardless of any allergy, known or unknown, that they (or my dependent) may have. To the extent that applicable statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of the Releasees.
