FSR-VCU Sarcoidosis Meet-Up

Come celebrate Sarcoidosis Awareness Month, with FSR, and Virginia Commonwealth University (VCU) to hear from Dr. Syed, patient speakers, and fun games and food. 

There is no fee for registration, but donations are welcome and all proceeds go directly to FSR to support outreach and awareness efforts.

*Photo Release*
All attendees and registrants (“Registrants”) to this live event hereby grant Foundation for Sarcoidosis Research (FSR) permission to use their likeness in a photograph in any and all of its publications, including website entries, without payment, or any other consideration. Registrants consent to and authorize the use and reproduction, in print or electronic format by FSR to use all and/or part of their story for promotional and educational uses, without compensation. Registrants agree that FSR may use said story about with or without a name for any lawful purpose including, but not limited to publicity, illustration, advertising, and web content.

Furthermore, Registrants hereby irrevocably authorize FSR to edit, alter, copy, exhibit, publish, or distribute stories and photos for purposes of publicizing FSR’s programs or for any other lawful purpose. In addition, Registrants waive the right to inspect or approve the finished product, including written or electronic copy, wherein their likeness appears. Additionally, Registrants waive any right to royalties or other compensation arising or related to the use of their stories or photographs.

All Registrants hereby hold harmless and release and forever discharge FSR from all claims, demands, and causes of action which the individual Registrants, their heirs, representatives, executors, administrators, or any other people acting on their behalf, or on behalf of their estate, have or may have by reason of this authorization.

*Release of Liability*
By registering for the Patient Meetup for Foundation for Sarcoidosis in Partnership with Virginia Commonwealth University Hospital("Event"), currently scheduled for April 12, 2026, 12pm-2:30pm ET at The Carillon Park shelter at Byrd Park, 1300 Blanton Ave. Richmond, VA 23221..

All attendees and registrants (“Registrants”) fully and completely waive and release the Foundation for Sarcoidosis Research ("FSR"), Virginia Commonwealth University Hospital, directors, officers, administrators, representatives and executors, past and present employees, volunteers, agents, supervisors, participants, all state and local governments, assigns, sponsors, their representatives and successors and other persons (collectively, the “Releasees”), from any and all claims, liabilities, damages, and/or causes of action arising out of an injury to Registrants and from any and all claims, liabilities, damages, and/or cause of actions arising from, or relating to, Registrants participation in the Event.

Inherent and Potential Risks
Registrants understand that the Event may involve physical activity involving walking or playing games (like cornhole, volleyball, etc.). Registrants understand that physical activity, by its very nature, carries with it certain inherent risks. Registrants assume all risks associated with participating in the Event relating to the risk of strenuous physical activity associated with walking, collisions with other participants, vehicles, and pedestrians, injury from sports or falling. Registrants acknowledge that they may incur minor injuries, major injuries, and catastrophic injuries including paralysis and death. Registrants assume all risks from contact with other participants and volunteers (including, but not limited to, contracting a disease), negligent or wanton acts of other participants and volunteers, any defects of conditions of the park (including uneven surfaces), failure of other participants, vehicles, and non-participants to observe traffic signals or laws, and the effects of weather including high heat, thunderstorms, lightning, precipitation, cold temperatures, high winds, and/or humidity.

All Registrants agree that they consume any food at their own risk and waive and release the Foundation for Sarcoidosis Research ("FSR"), Virginia Commonwealth University Hospital, directors, officers, administrators, representatives and executors, past and present employees, volunteers, agents, supervisors, participants, all state and local governments, assigns, sponsors, food donors, and their representatives and successors and other persons (collectively, the “Releasees”), from any and all claims, liabilities, damages, and/or causes of action arising out of an injury to themselves and from any and all claims, liabilities, damages, and/or cause of actions arising from, or relating to, Registrants consuming any food or drink provided by the facility. Registrants are solely responsible for any adverse health effects from food or beverage consumption, regardless of any allergy, known or unknown, that Registrants may have. To the extent that applicable statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of the Releasees.
